All Charm Locations in Mafia: The Old Country

In Mafia: The Old Country, charms are special items that make it easier in the missions and combat more effective. Collecting them all lets you customise your playstyle and gain the edge you need to survive Sicily’s dangerous underworld. This guide will help you find every single charm in the game, so you can make your character stronger.

Mafia: The Old Country All Charm Locations

In Mafia: The Old Country, charms give you special boosts through two types of items: Rosary Beads and Medallions. Medallions are your main power-up, but you can only wear one at a time, so choosing the right one matters. Beads offer smaller bonuses, but you can stack several of them to fine-tune your playstyle.

The total number of charms is 34. As you progress through the 14-story chapters, you can find the majority of the 22 of them by gathering San Celeste sculptures. The remainder can be obtained through special edition packs and pre-order bonuses, or they can be purchased from Pasquale’s store.

Starting Charms – Already in Your Pocket

When you begin your journey, you’ll already have two charms without lifting a finger:

  • Rusty Medallion – A simple medallion with no special powers.
  • Wooden Bead – Just a bead, no magic here either.

They’re unlocked by default, so you don’t have to search for them.

Chapter 1 – Sant’Agata’s Blessing

In Enzo’s living quarters, during the “Get ready for the meal” part of the mission, check the desk. You’ll find the Sant’Agata Medallion. When you’re close to death, this charm blesses you with a short burst of invulnerability just enough to claw your way back into the fight.

Chapter 2 – Fortune on Your Side

While heading to the Torrisi race tent, take a quick look at the wooden steps to the left of the tent. Here you’ll spot Fortunato, a bead that makes enemies drop more Dinari when looted. Perfect for anyone trying to stay flush with cash.

Chapter 3 – A Deadlier Throw & Faster Sharpening

Astuzia – On the mantel above an old garden fireplace during “Meet Luca at the stables.” It gives anything you throw a bigger area of effect.

Ingegnoso – In the abandoned farmhouse during “Pick up the rifle.” It lets you sharpen your blade much faster.

Chapter 4 – Silent Steps & Fully Loaded Guns

While sneaking into Messina’s camp during “Find Gennaro,” check the stone wall near the entrance for Fantasma. This bead quiets your footsteps, making stealth far easier.

Later in the same mission, during “Find a way out of the camp,” you can grab San Giuda, a medallion sitting on a wooden box. It ensures that any weapon you loot from enemies is fully loaded and ready to fire.

Chapter 5 – More Ammo for Your Rifle

During “Reach Ludovici’s study,” in Ludovici’s home, search for an elaborate cabinet. Caricato, a bead that boosts your rifle’s ammunition capacity, is inside. For marksmen, it can be a lifesaver.

Chapter 6 – Deadly Throws & Faster Aiming

Check for Abile, which increases the force of your thrown blades, on the wooden barrel close to the forger’s cell while releasing Giuseppe.

Acuto is later seen on a dressing table in D’Amico’s office during “Find the printing plates.” You can achieve complete targeting accuracy more quickly with this bead.

Chapter 7 – Better Loot & Quicker Bandaging

Astuto is seated at a table close to the warehouse door at the beginning of the chapter, during “Follow Leo.” When looting, this improves your chances of discovering beneficial consumables.

In one of the rooms you go through in the “Get a better view” portion, look for a tiny wooden container to pick up Medicale, which speeds up the bandaging process.

Chapter 8 – One-Shot Kills & Faster Looting

Look for San Bernardo da Corleone on a table in the printing press room during “Find the forger.” It offers you the opportunity to eliminate adversaries with a single blow.

You will find Collettore on a crate close to a boarded-up doorway in the “Find L’Ombra” area. This bead speeds up your looting, saving you valuable time when engaging in risky scavenging.

Chapter 9 – Faster Recovery & More Firepower

You’ll find three beads in this chapter:

During “Investigate Lorenzetti’s office,” Perseveranza was on Lorenzetti’s desk. expedites the healing process while you are near death.

During “Find a way into the foreman’s office,” Brutale is sitting on a bench close to the entrance to the foreman’s office. increases the damage of your ranged weapon.

Practicalo: During “Escape the Factory,” on a table behind a few crates on the ground floor of the facility. increases accuracy whether shooting from the hip or when blind..

Chapter 11 – More Ammo & Clearer Vision

In “Follow Don Torrisi,” search for Economico on the right side of the hallway to find more ammunition.

In the “Follow Leo” section, look for Valore on a table beside a blue door. By reducing the blur from approaching gunfire, it aids in maintaining a clean aim.

Chapter 12 – A Sharper Eye

While making your way to the Royal Box, after pushing a cart into the opera house, check the first table you see for Paziente. This bead improves your aiming accuracy.

Chapter 13 – Reload in a Flash

In the area where you fight Mazzone during “Regroup with Cesare,” there’s Destrezza sitting on a barrel. This bead speeds up reload times for all your guns.

Store-Bought Charms

Pasquale’s store sells a variety of useful beads and medallions for Dinari:

  • San Francesco d’Assisi (Medallion) – extends the life of your sword (800 Dinari)..
  • Munito – Carry more pistol ammo (500 Dinari).
  • Potente – More shotgun ammo capacity (500 Dinari).
  • Preparato – More bandages (750 Dinari).
  • Explosivo – Carry more grenades (600 Dinari).
  • Fortezza – Reduces damage during duels (1,200 Dinari).

Special & Bonus Charms

Some charms are exclusive to pre-orders, deluxe editions, or special conditions:

  • Lupara (Medallion) – The Lupara shotgun (Soldato pack pre-order bonus) allows for faster shooting and reloading.
  • Immortale (Medallion) – Chance to automatically avoid a bullet (Padrino pack in the deluxe edition)
  • Velocita (Medallion) – Better handling, suspension, and damage resistance of car (Gatto Nero pack in the deluxe edition).
  • Lincoln’s Charm (Medallion) – Quickens struggle and choking motions. A 2K account that has played Mafia III: Definitive Edition must be linked in order to unlock it.

Every charm in Mafia: The Old Country has a purpose, but let’s face it, some are unquestionably more potent than others. You can only wear one bead and one medallion at a time at first. Visit Pasquale’s store as you advance to obtain the Silver and Gold improvements. You will have much more flexibility to mix and match your favourite beads with them since they will allow you to equip up to three extras.
