In the thirteenth mission of Mafia: The Old Country, Enzo has been instructed to deal with Matteo Costa. Since the story itself does not branch off depending on your choice, there is a cool little reward for being merciful. If you take too long hesitating instead of killing him, you will unlock the secret “Man of Honor” achievement.
By this time, Don Spadaro and his enforcer, Raffaele Fontanella, have already been taken out in chapter 12. There are not many of the captains remaining that are loyal to a deceased Don. Enzo, along with Cesare and Tino, take out Ignazio, Dino Mazzone, and Domizia Caccini and then finally corner Costa inside a church.
Inside, the pressure builds—Cesare and Tino insist you pull the trigger, while Father Ciccone pleads for mercy. Costa himself begs for his life, promising he’ll leave Sicily and never return if spared.
Kill or Spare: What Each Choice Does
- Killing Costa: If you go through with it and shoot him, Costa dies immediately. Father Ciccone reacts with grief, but the story rolls on without any major changes.
- Letting Him Live: If you refuse to shoot for around 30–35 seconds, Cesare will step in and execute Costa himself. This path unlocks the “Man of Honor” achievement, rewarding you for showing restraint.
Even though the end result is the same, sparing Costa fits better with Enzo’s mindset at this stage. He’s tired of the Mafia life, thinking about escaping to America with Isabella, and less willing to kill blindly.
