This step-by-step guide walks you through starting the quest, locating key NPCs, defeating the Plague Wraith, and earning game-changing rewards that boost both survival and offensive power.
Step 1: Start the Quest

- Travel to the second map, Cuanacht.
- Go to the main city area and locate the hospital.
- Speak with the patient named Ignatius to begin the quest “Perinde Ac Cadaver” (Latin for “just like a corpse”).
Step 2: Reach the Toll House

- Head east from the hospital to reach the Toll House, a large castle full of infected enemies.
- Fight through the castle to reach the top floor.
- Use the elevator at the top. (Note: The elevator may be bugged; leave and re-enter the area if needed.)
- Take the elevator down to meet The One.
Step 3: Receive and Deliver the Marks
The One will instruct you to deliver the Mark of the Plague to two individuals:
1. Una
- Found in the plague-infected district of the city.
- Important: Complete her side quest before marking her for death.
2. Ignatius
- Return to the hospital and deliver the mark to him.
Step 4: Use the Final Mark
- Return to The One.
- Use the final Mark of Death on him.
- This action will transport the player to a bloody alternate version of the asylum.
Step 5: Defeat the Plaguewraith & Meet NPC
- Battle and defeat the Plaguewraith, a challenging boss enemy.
- After the boss fight, an important story-related NPC will appear.

Step 7: Claim the Rewards
After the dialogue, the player is transported back to the Toll House.
